Dr. Richard Cook is a research scientist, physician, and student of resilience engineering for safety in complex risk-critical worlds, and co-author of Behind Human Error (2010). Richard is emeritus professor of healthcare systems safety at Sweden’s KTH. He is one of the team at Adaptive Capacity Labs.
